چکیده

Non-invasive prediction of optimal targets for e cient radiofrequency ablation is a major challenge in the treatment of ventricular tachycardia. Most of the related modelling work relies on magnetic resonance imaging of the heart for patient-speci c personalized electrophysiology simulations. In this study, we used high-resolution computed tomography images to personalize an Eikonal model of cardiac electrophysiology in seven patients, addressed to us for catheter ablation in the context of postinfarction arrhythmia. We took advantage of the detailed geometry offered by such images, which are also more easily available in clinical practice, to estimate a conduction speed parameter based on myocardial wall thickness. We used this model to simulate a propagation directly on voxel data, in similar conditions to the ones invasively observed during the ablation procedure. We then compared the results of our simulations to dense activation maps that recorded ventricular tachycardias during the procedures. We showed as a proof of concept that realistic re-entrant pathways responsible for ventricular tachycardia can be reproduced using our framework, directly from imaging data.

Inverse solution mapping of epicardial potentials: quantitative comparison with epicardial contact mapping.

BACKGROUND Catheter ablation of ventricular tachycardia (VT) is still one of the most challenging procedures in cardiac electrophysiology, limited, in part, by unmappable arrhythmias that are nonsustained or poorly tolerated. Calculation of the inverse solution from body surface potential mapping (sometimes known as ECG imaging) has shown tremendous promise and can rapidly map these arrhythmias...

Role of high-resolution image integration to visualize left phrenic nerve and coronary arteries during epicardial ventricular tachycardia ablation.

BACKGROUND Epicardial ventricular tachycardia (VT) ablation is associated with risks of coronary artery (CA) and phrenic nerve (PN) injury. We investigated the role of multidetector computed tomography in visualizing CA and PN during VT ablation. METHODS AND RESULTS Ninety-five consecutive patients (86 men; age, 57 ± 15) with VT underwent cardiac multidetector computed tomography. The PN dete...

Impact of New Technologies and Approaches for Post-Myocardial Infarction Ventricular Tachycardia Ablation During Long-Term Follow-Up.

BACKGROUND During the past years, many innovations have been introduced to facilitate catheter ablation of post-myocardial infarction ventricular tachycardia. However, the predictors of outcome after ablation were not thoroughly studied. METHODS AND RESULTS From 2009 to 2013, consecutive patients referred for post-myocardial infarction ventricular tachycardia ablation were included. The end p...

Ablation of ventricular tachycardia in chronic chagasic cardiomyopathy with giant basal aneurysm: Carto sound, CT, and MRI merge.

Chronic chagasic cardiomyopathy (CCC) is a parasitic disease that presents with life-threatening ventricular arrhythmias, dilated cardiomyopathy, or sudden death. Basal and posterior wall motion abnormalities and left apical aneurysms are common. We present a report of a patient with CCC, sustained ventricular tachycardia (VT) refractory to amiodarone 400 mg/day and carvedilol 25 mg/day BID wit...

Long-Term Outcomes of Catheter Ablation of Ventricular Tachycardia in Patients With Cardiac Sarcoidosis.

BACKGROUND Catheter ablation (CA) of ventricular tachycardia (VT) in patients with cardiac sarcoidosis can be challenging because of the complex underlying substrate. We sought to determine the long-term outcome of CA of VT in patients with cardiac sarcoidosis. METHODS AND RESULTS We enrolled 31 patients (age, 55±10 years) with diagnosis of cardiac sarcoidosis based on Heart Rhythm Society cr...
